The app maintains digital patient records, tracks follow-ups, and sends timely alerts to ensure treatment adherence. For patients unable to visit the clinic, Dr. Udesh and his team monitor health via the app and conduct home visits. This initiative brings healthcare directly to villagers’ doorsteps, ensuring continuous and timely care. The project exemplifies how technology can strengthen rural healthcare, aligning with India’s Digital India vision and promoting better management of chronic diseases.
